Access Patient Profiles
Our Physician Portal allows you to log in to see a holistic view of your patients with the accessibility to view their full visit history in one single system. Note: The information provided in this dashboard is not intended for diagnosis or treatment of any disease.
Patient Privacy
- Some information in the site dashboard is considered protected health information under the Health Insurance Portability and Accountability Act of 1996, as amended (HIPAA). All site dashboard users are required to follow HIPAA and other applicable federal and state patient privacy and security laws, rules and regulations.
- Site dashboard users will only have access to information regarding the site’s own patients.
- Secure access to the physician portal and the site dashboard is granted by CorEvitas and may be revoked at any time in CorEvitas' sole discretion.
- Login credentials are unique to the user and may never be shared.
- Please notify CorEvitas at ra@corevitas.com of any changes in site location, providers or staff such that records and access may be updated.
